SmartEven is an innovative e-Health tool to support the management of chronically ill patients, de-hospitalization and continuity of care. Thanks to the data acquired through the use of the app and electromedical equipment, healthcare personnel can in any place and at any time consult the history of the parameters of each patient, detect any anomalies, verify the correct intake of drugs, communicate with the patient, family members and the entire care team. Supporting the patient in acquiring / transmitting the clinical data necessary to monitor the course of the disease also helps reduce the risks associated with complications, acute conditions and re-hospitalizations.

IRCCS Monzino Cardiology Center in Milan has chosen SmartEven to enrich Monzino DIGITAL, its suite of digital services and information to create a modern direct patient / hospital line. Through a continuous monitoring system, in fact, SmartEven represents a real hospital service brought to the patient's home.

The system fully complies with the provisions of the Regional Law of 11 August 2015 regarding alternative organizational models for the management of chronic subjects: the patient is the first actor, together with his family and health personnel, in the optimal management of the disease.
